Two Simple Ways to Give

1. Make Northwest Colorado Visiting Nurse Association a designated beneficiary of all or a percentage of your
   • IRA or Retirement Plan
   • Bank Account
   • Investment Account
   • Life Insurance Policy
2. Include a simple bequest to Northwest Colorado Visiting Nurse Association in your will using the following language:
   "I give, devise and bequeath to Northwest Colorado Visiting Nurse Association, 940 Central Park Dr., Steamboat Springs, CO 80487
   Federal Tax ID# 84-0564998, and its successors and assignees:
   
   • the sum of $___________________.    (specific bequest)
   • % of my estate.    (percentage of estate)
   • all of the residue of my estate.    (residual bequest)
