Hospice and Palliative Care

Hospice and Palliative Care offers terminally ill patients and their families complete support and skilled personalized care at home. When a patient realizes that cure is not probable and treatment is aimed at providing comfort and enhancing quality of life, the hospice team can begin to help ease the physical, emotional and spiritual pain for patients and families.

Our Services Include:

• Scheduled visits by the Hospice team including Medical Director, Nurse, Social Worker, Chaplain, Home Health Aide, Physical/Occupational/Speech Therapist, dietician and volunteers
• Coordination with the patient's physician and with other community resources
• Assistance with legal and financial concerns
• Spiritual counseling, if desired
• Professional staff on call 7 days a week
• Teaching the family to care for the patient at home
• Care after the death to guide loved ones through the grief process for one year
