Chronic Disease Self-Management Program

Healthier Living is a 6-week self-management program designed to help adults age 18 and older live healthier and more active lives. Learn strategies for successful goal setting that can help you manage health challenges more positively with better results.

Healthier Living Diabetes applies the same principles and practices as traditional Healthier Living classes, with emphasis on strategies for coping with the symptoms and challenges of living with diabetes.

Cost for all Healthier Living 6-week workshops is $20 for books, CD and materials. Partners and caregivers are encouraged to attend classes.
