We are your local community health center providing medical services for people of all ages, all income levels, all insurance status, on a sliding fee scale. As the only federally qualified Community Health Center in Northwest Colorado, we utilize many resources to help keep your costs as low as possible. We pro-actively ensure that everyone has access to quality health care through affordability, creative staffing and careful management.

We do not deny health care based on financial limitations.

Hours of Operation

Monday from 8am to 7pm
Tuesday though Friday 8 am to 5pm
After hours and on weekends Health Center clients have access to professional services by calling the Health Center number.

Payment and Billing Question

All services are offered on a sliding fee scale based on income for the uninsured. We accept most private insurance, Medicare, Medicaid and CHP+. Please be prepared to pay (or co-pay) for services at the time of your appointment. We accept cash, check, Visa, and MasterCard.
