Birth and Death Certificates

Hours of Operation are 9:00 am to 4:00 pm Monday-Friday in Routt and Moffat Counties.

Vital records in the state of Colorado are confidential. Applicants requesting certified copies must provide acceptable identification as well as proof of relationship or direct and tangible interest. Applications and additional information are available below or by contacting the VNA directly at:

Birth Certificates:
Birth Certificates for individuals born anywhere in the state of Colorado may be obtained from the VNA offices in both Routt and Moffat county by completing the required application and providing acceptable identification as well as proof of relationship or direct and tangible interest. Birth Certificates are $17.75 for the first copy and $10.00 for each additional copy. Applications will be processed within a 48 hour time frame. Birth Certificates are not available on a walk-in basis.

Death Certificates:
VNA maintains the records of Routt and Moffat county deaths only. ALL DEATH RECORDS HAVE TO OBTAINED FROM THE COUNTY IN WHICH THE DEATH TOOK PLACE. Death Certificates may be obtained from the VNA offices in both Routt and Moffat county by completing the required application and providing acceptable identification as well as proof of relationship or direct and tangible interest. Death Certificates are $17.00 for the first copy and $10.00 for each additional copy. Applications will be processed within a 48 hour time frame. Death Certificates are not available on a walk-in basis.
